SUBCHAPTER D. ADMINISTRATION OF ANESTHESIA

The board by rule shall require a dentist who applies for the issuance or renewal of a permit under this subchapter to include in the application a statement indicating whether the dentist provides or will provide a permitted anesthesia service in more than one location.
