Tex. Special District Local Laws Code § 1100.152: BALLOT.

The ballot for the election must be printed to permit voting for or against the proposition: "Expanding the South Randall County Hospital District to include all of the territory in Randall County that is not included in the City of Amarillo Hospital District, the assumption by the additional territory of its proportionate share of the district's outstanding debt, and the imposition of a tax not to exceed 75 cents on each $100 of valuation on all taxable property in the expanded area of the district."
