Tex. Transportation Code § 548.404: APPLICATION FOR CERTIFICATION AS INSPECTOR.

An application for certification as an inspector shall:

(1) be made on a form prescribed and provided by the department; and

(2) state:

(A) the name of the applicant;

(B) the address of the applicant's residence and place of employment;

(C) the applicant's driver's license number; and

(D) other information required by the department.
