18VAC10-20-660: Change of status

A. Any changes of status, including change in entity, name (including assumed names), address, place of business, or responsible persons at each place of business, must be reported to the board by the registered entity within 30 days of the occurrence. In addition, any licensed or certified employee responsible for such practice must notify the board in writing of any changes of employment status within 30 days of the change.

B. In the event there is a change in the responsible person, whether the change is temporary or permanent, the registration will be automatically modified to be limited to that professional practice permitted by the remaining licensed or certified employees, or will be automatically suspended until such time as the entity comes into compliance with this chapter.
