Va. Code § 38.2-1303: Printed forms to be filed by insurers; certificates to domestic insurers

A. The Commission shall be responsible for prescribing the type of blank or may prepare and distribute printed forms or blanks to licensed insurers for statements, reports, schedules or exhibits required by law or order.B.
