N.C. Gen. Stat. § 86B-24: Application for examinations; payment of fee.

Each applicant for an examination shall do all of the following:

(1) Make application to the Board on forms prepared and furnished by the Board, and the application shall contain proof of the particular qualifications of the applicant.
