Okla. Stat. tit. 63, § 63-7100.4: Authorization form for exchange of health information -

Instructions.

A. The State Board of Health shall adopt and distribute a

standard authorization form and accompanying instructions for use in

obtaining authorization for the exchange of health information.

B. The authorization form adopted and distributed by the Board

shall comply with all applicable federal and state privacy and

privilege laws.
