Okla. Stat. tit. 63, § 63-7100.5: Acceptance and use of form

A. A health care entity shall accept the authorization form

adopted and distributed by the State Board of Health as a valid

authorization for the exchange of health information.

B. A health care entity is not required under this act to use

the authorization form adopted and distributed by the Board.
